21 january 1993 – italian development merlin pp2 crashed near novara-cameri airfield in italy after uncommanded application of rotor brake in flight. 4 people killed.
7 april 1995 – british development merlin pp4 (zf644) crashed while in-flight yeovil, england. 4 crew members escaped helicopter during descent before hit ground.
20 august 1996 – italian development merlin pp7 (i-hioi) damaged in accident when turned on after tail rotor drive failed on landing. helicopter repaired.
27 october 2000 – royal navy merlin (zh844) ditched near isle of skye, scotland after hydraulic fire caused rotor brake being partially engaged.
30 march 2004 – royal navy merlin (zh859) crashed on take-off rnas culdrose due tail rotor hub cracking.
15 november 2007 – during embarkation medical evacuation on são jorge island, azores, portuguese air force merlin caused injuries 5 people when unexpectedly climbed 1 metre before pilot recovered control. portuguese air force spokesperson later stated kind of incident unheard of.
14 october 2009 - british royal navy eh101 merlin landed short of designated landing pad on canadian forces hmcs montreal. cause determined pilot error. substantial damage helicopter no injuries.
9 november 2009 - british royal air force aw101 merlin hc3 crashed @ airfield during night time training exercise near holtville, ca in preparation deployment afghanistan. aircraft fell on side following hard landing. helicopter 1 of 4 merlins deployed el centro, ca part of exercise merlin vortex.
11 october 2014 - royal danish air force aw101 suffered extensive damage after rolling on after experiencing pilot-induced oscillation following landing in afghanistan. no casualties.
18 august 2017 japan maritime self-defense force ch-101 flipped during cargo hauling exercise, injuring 4 servicemen , causing substantial damage. helicopter had been assigned icebreaker shirase expedition antarctica.
24 november 2017 norwegian armed forces helicopter, delivered 1 week earlier, tipped on during test program while on ground on stavanger airport, sola, norway. 2 pilots , 4 ground crew present, no injuries reported.
